What are the reasons for international differences in estimated CKD prevalence?

Vianda S. Stel and her colleagues from the ERA-EDTA Registry studied the association of risk factors for chronic kidney disease (CKD) and examined potential reasons for international differences in estimated CKD prevalence. They found substantial variation in risk factors for CKD such as in the prevalence of diabetes mellitus, obesity, raised blood pressure, physical inactivity, current smoking and salt intake per day. But differences in CKD prevalence might also be due to heterogeneity in study methods.  The authors conclude that a robust public health approach to reduce risk factors in order to prevent CKD is needed and will have co-benefits for other non-communicable diseases.

How to design a randomized water intake trial in ADPKD?

Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited renal disease, affecting 12.5 million people worldwide. There is an urgent need for trials to evaluate the optimal hydration strategy in patients with ADPKD, because many clinicians encourage ADPKD patients to increase their daily water intake to 2–4 L, but there is no existing evidence to support this promotion of high water intake. This study group from Cambridge (United Kingdom) conducted a survey of patients with ADPKD to inform the design of a randomized feasibility trial of high versus standard water intake. In this paper potential barriers and methodological issues of a water intake trial are carefully discussed.
